Jon Kabat-Zinn arrived at Mindfulness-Based Stress Reduction from an unusual pairing of training: a doctorate in molecular biology from MIT, earned under Nobel laureate Salvador Luria, alongside years of personal study in Buddhist meditation and yoga pursued outside his scientific career. In 1979, at the University of Massachusetts Medical Center, he opened what he called the Stress Reduction and Relaxation Program, offering an eight-week course to patients referred by their physicians for chronic pain and stress-related conditions that conventional treatment had not resolved.

What made the program distinctive, and what let it travel so far beyond one hospital, was the decision to strip the Buddhist practices it drew on of explicit religious and cosmological framing and present them instead in the language of attention, stress physiology and patient self-management, a deliberately secular clinical adaptation rather than a course in Buddhism. The manualised, repeatable eight-week format, sitting meditation, a body scan practice, gentle Hatha yoga, made it straightforward to teach consistently and, crucially, to study. Kabat-Zinn's own books, especially Full Catastrophe Living in 1990, and the growing clinical research literature carried the program, renamed Mindfulness-Based Stress Reduction, out of that one medical center and into hospitals, clinics, schools and corporate wellness programs worldwide, and made it the template a whole family of later mindfulness-based clinical programs were built to follow.
